Percutaneous endoscopic gastrostomy (normally called PEG) is a minimally invasive procedure which provides a crucial nutritional pathway without the need for surgical intervention or general anesthesia. It is particularly beneficial for patients who unable to tolerate oral feeding due to various medical conditions. Common indications includes neurological disorders causing long-term dysphagia (such as brainstem inflammation, tumors, or post-stroke impairment), severe systemic illnesses requiring nutritional support, and complications from head and neck surgeries necessitating prolonged feeding assistance.
PEG procedures is conducted through an endoscope, facilitating access even in cases of narrow or obstructed passages in the upper gastrointestinal tract. The technique offers simplicity, safety, and minimal complications, making it the preferred method for establishing gastric feeding tubes, which normally has three different techniques:
1. Pull Method: It utilizes a loop device inserted through the endoscope to grasp and pull a feeding tube into place, currently widely used.

2. Push Method: It involves the use of a push system with a Seldinger needle to insert and advance the feeding tube under endoscopic guidance. It can be used for patients with esophageal obstruction who cannot use the pull method.

3. Direct Percutaneous Method: It's a straightforward approach utilizing a gastrostomy tray, ideal for straightforward cases.
Each technique aims to secure a feeding tube through a small incision in the left upper abdomen, corresponding to the lower gastric anterior wall near the angle. Careful preparation and sterile technique are crucial to minimize complications such as infection, bleeding, or inadvertent organ injury.
ABout Post-Procedure Management
After PEG placement, patients are initially kept nil by mouth to allow for recovery, followed by gradual introduction of nutrition through the tube. Vigilance for complications like infection or tube dislodgement is essential, with prompt intervention to manage any issues that may arise. When the stoma has healed, a balloon replacement tube or button balloon tube can be used.
