About irrigation pressure
Irrigation pressure is related to the inflammatory effect stage after skin and subcutaneous tissue trauma. Traditional gravity irrigation is difficult to completely remove fibrin, bacteria, foreign matter or necrotic cell fragments covering the wound.
Pressure irrigation can effectively remove various particles and blood clots in the wound by using the power of water flow, and achieve the purpose of peeling off the fibrin membrane on the wound surface to prevent wound infection and promote wound healing.
Pound force per square inch (PSI) is usually used as the unit of measurement for irrigation pressure. Usually, the irrigation pressure is graded as follows: < 5 PSI is low pressure, 5~8 PSI is medium pressure, and >8 PSI is high pressure. At present, most scholars believe that the irrigation pressure of 5~8 PSI is suitable for the irrigation needs of most wounds.
Therefore, for patients with mild contamination and injury < 6 hours, medium pressure (5~8 PSI) can be considered for wound irrigation. High-pressure irrigation is more suitable for patients with severe contamination and delayed medical treatment after injury (>6 hours). High-pressure irrigation should be used with caution for open fractures, and 12 PSI can be used as the standard for open fracture wound irrigation.
