Sometime, the healthcare professionals and caregivers ask whether to fill the balloon with air, distilled water, or saline solution. It's a very important question for maintening the feeding tubes. We normally suggest to use the distilled water.
1. Why Sterile Distilled Water Is Recommended?
The balloon of a gastrostomy tube(replacement tube or balloon tube) should be inflated with sterile distilled water, not saline or any others, because of the risk of crystallization. Saline contains sodium chloride, which will form crystals over time. These crystals have the risk of blocking the balloon's lumen, and making it difficult or even impossible to withdraw the contents from the balloon. Furthermore, the balloon's structural integrity may be jeopardized by the presence of crystalline materials within, which could result in an early failure.
Contrast media or other solutions should be avoided for the same reason, which is increasing the possibility of blockage due to crystallize. The blockage not only makes it difficult to deflate the balloon, but can also lead to complications during the removal or replacement of the gastrostomy tube.
2. The Risks of Using Air
Another alternative is using air, which we don't recommend. The reason is that the nature of air molecules is smaller than water molecules. Silicone, the material of gastrostomy tube balloons, is permeable to gases. Therefore, if air is used to inflate the balloon, it will leak through the silicone membrane, leading to the balloon's deflation over time. The deflation will cause the gastrostomy tube to become dislodged, which will be a significant risk for the patient, requiring urgent medical attention to replace the tube.
3. How to mainten the balloon?
Even used sterile distilled water, it's also important to note that the balloon still have posibble to leakage over time. Although the water molecules are larger than air molecules, which also can pass through the silicone wall of the balloon, and albeit at a slower rate. For this reason, regular maintenance is crucial.
Normally, it is recommended to replace the water inside the balloon every 7-10 days. This practice ensures that the balloon remains adequately inflated, reducing the risk of tube dislodgement. Regularly replacing the water also helps maintain the balloon's integrity and performance, as stagnant water inside the balloon could potentially harbor bacteria or lead to biofilm formation.
4. Practical Tips for Balloon Maintenance
1) Using Sterile Equipment: To minimize the risk of introducing infections, when inflating or deflating the balloon, please always using a sterile syringe.
2) Checking Balloon's Integrity: During routine checks, deflate the balloon and then reinflate it to the recommended volume. This helps to ensure the balloon's integrity and that it is functioning correctly.
3) Avoiding Overinflation: Overinflating the balloon beyond the recommended volume might cause unnecessary stress on the balloon material, leading to shorten the balloon's using life. So healthcare have to follow the manufacturer's guidelines for the correct volume.
4) Monitoring the Signs of Leakage: If the gastrostomy tube looks like loose or if the patient feel uncomfortable, it might be a sign of leakaging of the balloon. The healthcare/doctors need to check the balloon immediately or replace the water as needed.
5) Strengthening patient Education: Ensuring that the healthcare and patients understand how to safely check and replace the water in the balloon, which can prevent complications and prolong the life of the gastrostomy tube.
